The Pennyrile District Health Department is accepting applica­tions for a full-time, partial year (10 month position) Local Health Nurse II to be housed in the Trigg County Middle School, pending signed contract with school.  

 

General Duties Include:  conduct nursing assessment for student, families, and school personnel as appropriate using Core Clinical Services Guide (CCSG) services guidelines, develop and implement a plan of care based upon identified health risks, provide basic nursing care to the client consistent with CCSG and Nursing Scope and Standards of Practice, established policies, procedures, and licensing laws, document services provided to the client according to standards of the CCSG and Administrative Reference (AR), maintain confidentiality and privacy according to guidelines, work closely with school interdisciplinary team and apply problem solving strategies to accomplish job tasks, participate in specialized or program-specific clinics, participate in educational counseling and teaching activities for individual students and educators, may assist coordinator of a specific program and offer mentoring to staff.

 

Minimum Education, Training or Experience:  See special requirements and one year of registered nurse (RN) experience.

 

Substitution for Education, Training or Experience:  BSN or Master’s Degree in Nursing, Nursing Administration, Nursing Education, or Public Health may substitute for one year of experience.

Salary:  Beginning hourly rate is an above-minimum rate of $20.81 per hour, negotiable with additional experience. Grade 18.

General Duties Include:  With experience, judgment and skill, interview client to address the presenting/initial reason at school site and subsequent willingness for services(s) offered and indicated. Conduct nursing assessment, as appropriate, of client using Core Clinical Services Guide (CCSG) service guidelines to obtain and document pertinent physical, social, behavioral, environmental and lifestyle factors that influence health. Obtain basic specimens, as directed, and utilize the Division of Laboratory Services Guidelines for proper laboratory/specimens handling and packaging. Record results, as reported by laboratory, on client chart. Perform preliminary assessment and interpretation of tests to identify health problems, report assessment to supervisor. Describe or refer services that are available and eligibility criteria as appropriate. Compile data for periodic assessments of client physical condition and evaluation of the effectiveness of health care procedures, approaches and programs in conjunction with other members of the interdisciplinary treatment or nursing care team. Under the supervision of a registered nurse, evaluate assessment data and develop a nursing care plan. Incorporate a plan of care into the client medical record. Periodically update plan of care based on follow-up visits and services provided and compliance of the client. Initiate appropriate teaching and demonstration of care based on assessment with emphasis on promotion and restoration of health

 

Minimum Education, Training or Experience:  Graduate from a school of practical nursing, college, or community and/or technical college, which meet the requirements set by Kentucky State Law. One year of nursing experience as a licensed practical nurse.

 

Special Requirements:  Current and active Kentucky Licensed Practical Nurse License.
