OPEN POSITIONS:


HANDS HOME VISITOR

The Pennyrile District Health Department is accepting applications for a full time Health Access Nurturing Development Services (HANDS) Home Visitor who will serve families in Caldwell, Crittenden, and Livingston County. The starting salary is $18 per hour, negotiable with additional experience. Benefits will be offered. Paper applications and a full listing of qualifications may be obtained at the Caldwell, Crittenden, Livingston, Lyon, or Trigg County local health departments.  

The HANDS Home Visitor will be responsible for but not limited to the following general duties:

  • Conducting parent visit assessment/survey within 30 days of receiving, after-reviews of prenatal or birth records from the hospital or clinic to determine eligibility for early childhood education and family support services of infants and toddlers from birth up to three years of age
  • Conducting interviews to promote outreach activities in family homes or community settings to educate parents in coping techniques
  • Accessing resources or assistance by means of referral for appropriate education and nutrition of HANDS program participants
  • Distributing information materials that meet program guidelines
  • Keeping accurate logs of service visits, schedules and charting, preparing reports and analyses set forth in HANDS program policy and guidelines
  • Interpreting laws, regulations, policies, and procedures 
  • Providing administrative assistance or guidance to departmental staff including but not limited to: monitoring, evaluating, and participating in modifications regarding program requirements 
  • Reviewing records and reports submitted by program requests or agency staff and ensuring compliance with federal and state law regulation and policies  
  • Conducting ongoing home visitations

Minimum Education, Training, and Experience:  Bachelor’s Degree in Social Work, Nursing, Early Childhood Education, or Elementary Education OR Bachelor’s Degree in Social or Behavioral Science or related field with one year experience performing case management services, OR a Master’s Degree in Human Services may substitute for the one year of experience  

Substitution for Education:  Associate Degree in Early Childhood Development and two years of Public Health, early childhood, or education experience OR Associate Degree in Nursing with licensure in Kentucky. Special Program Requirements:  Meets education requirement per 902 KAR 4:120. Applicant will be required to possess/maintain a valid driver’s license, vehicle insurance, and reliable transportation.

LOCAL HEALTH REGISTERED NURSE

The Pennyrile District Health Department is accepting applications for a full time Local Health Registered Nurse I for Lyon County. The starting salary is $22 per hour, negotiable with additional experience. Benefits will be offered. Paper applications and a full listing of qualifications may be obtained at the Caldwell, Crittenden, Livingston, Lyon, or Trigg County local health departments. 

The Local Health Registered Nurse I will serve under the direction of the Director of Nursing. 

General duties include but are not limited to: 

  • Providing basic entry level nursing care and assessments to all clients consistent with the Public Health Practice Reference, Nursing Scope, and Standards of Practice
  • Establishing policies, procedures, and licensing laws
  • Documenting services provided to clients according to standards of the Core Clinical Service Guide (CCSG) and Administrative Reference (AR)
  • Maintaining confidentiality and privacy according to guidelines 
  • Developing and conducting educational counseling and teaching activities for individual clients and families  
  • Utilizing basic awareness of community resources
  • Completing required trainings  
  • Maintaining RN licensure in Kentucky
