OPEN POSITIONS:


Medical Nutrition Therapy (MNT) Nutritionist I

The Pennyrile District Health Department is accepting applications for a full time Medical Nutrition Therapy (MNT) Nutritionist I to be housed within the Pennyrile District Health Department. The starting salary is $22 per hour, negotiable with additional experience. Benefits will be offered. Paper applications and a full listing of qualifications may be obtained at the Caldwell, Crittenden, Livingston, Lyon, or Trigg County local health departments.

The MNT Nutritionist I will serve under the direction of the Director of Nursing.

General duties include but are not limited to:

· Perform work of moderate to complex nutrition case management of medically high-risk individuals including physician prescribed dietary and nutrition regimens and intensive counseling (Medical Nutrition Therapy/MNT)

· Perform work of moderate to complex management in the application of nutrition judgment and skill

· Participate in on-the-job training activities to deliver basic nutrition and MNT services (group and/or individual contact) based upon programmatic skills, competencies and requirements

· Assess basic and/or MNT nutrition status of individuals and develop a basic and/or MNT nutrition plan appropriate for each client or client group according to programmatic criteria

· Interpret current research regarding basic nutrition and MNT and provide technical assistance and consultation for other health care professionals

· Obtain a National Provider Identification (NPI) number for reimbursement for MNT services

· Assess community nutrition needs.

· Develop and implement community events and programs

· Collaborate with state and community agencies

· Plan and deliver in-service training and education for public health personnel

· Maintain quality of services in accordance with agency policies and as outlined in the Core Clinical Service Guide (CCGS) and Administrative Reference (AR)

· Provide general technical assistance and consultation to schools, health professionals, co-workers, community organizations, and the general public on nutrition.

Special Requirements: Must be a Licensed Dietitian (LD) as provided by the Kentucky Board of Licensure and Certification as provided in KRS 310.021 or be certified as a Certified Nutritionist (CN) by the Kentucky State Board of Licensure and Certification as provided in KRS 310.031.

Health Educator I

The Pennyrile District Health Department is accepting applications for a full time Health Educator I to be housed within the Pennyrile District Health Department. The starting salary is $20 per hour, negotiable with additional experience. Benefits will be offered. The Health Educator I will serve under the direction of the Director of Nursing.

General duties include but are not limited to:

· Attend orientation sessions to develop an understanding of the role of the clinic in health assessment problems

· Meet with members of the community to establish a working relationship and become familiar with available community resources

· Assist in exchanging information with various organizations and agencies concerning health needs of the community and assist in assembling materials to be utilized

· Gather health information for the public relative to the specialized program

· Distribute health education materials by a variety of methods

· Provide services according to the guidelines set by the Core Clinical Service Guide (CCSG) /Administrative Reference (AR)

· Participate as a team member, having a basic understanding of the team process.

· Demonstrate an awareness of public service announcements (PSA) and assist in writing public service announcements for radio and television

· Submit written material to assigned supervisor for approval

· Exchange information with various organizations concerning health education

· Oversee the dissemination of health information to the public utilizing radio, TV, social media and newspaper contacts

· Collect and analyze data for health program needs

· Publish and maintain social media outlets for the agency

Minimum Education, Training or Experience: Bachelor’s degree and one (1) year of health related or education experience.

Substitution for Education, Training or Experience: Master’s Degree or Bachelor’s Degree in health or human services fields will substitute for one (1) year experience required. A professional license in the health or human services field will substitute for an additional year of experience.

Local Health Registered Nurse I

The Pennyrile District Health Department is accepting applications for a full time Local Health Registered Nurse I for Lyon County. The starting salary is $22 per hour, negotiable with additional experience. Benefits will be offered. The Local Health Registered Nurse I will serve under the direction of the Director of Nursing.

General duties include but are not limited to:

· Providing basic entry level nursing care and assessments to all clients consistent with the Public Health Practice Reference, Nursing Scope, and Standards of Practice

· Establishing policies, procedures, and licensing laws

· Documenting services provided to clients according to standards of the Core Clinical Service Guide (CCSG) and Administrative Reference (AR)

· Maintaining confidentiality and privacy according to guidelines

· Developing and conducting educational counseling and teaching activities for individual clients and families

· Utilizing basic awareness of community resources

· Completing required trainings

· Maintaining RN licensure in Kentucky
